1. Time Period
Specify whether the time period is P-period-to-date, Q-quarter-to-date, or Y-year-to-date.
2. Tax District
Specify tax district (2 characters). The entry must be a valid tax district.
3. Employee
Specify the employee for which unemployment history is to be recorded.
4. Earnings
Specify the total earnings of the employee (+/-9999999.99).
5. Taxable Earnings
Specify the taxable earnings of the employee (+/-9999999.99).
